- To provide our Services: We process Personal Information in order to provide Fast 602 LLC’s Services, which includes the creation of customer accounts and authenticating logins, communication with you, facilitating communications (including requests for 602a forms) between users and the medical professionals responsible for completing the forms, providing users with access to their health information, and processing payments.
- To analyze and improve our Services: We constantly work to improve and provide new tools and Services. We analyze use of our website and platform to improve the customer experience or assess our marketing campaigns.
Sharing Your Information:
We do not share your Personal Information with third parties, except as described in Fast 602 LLC’s privacy policy or as otherwise disclosed to you. This notice describes how your medical information may be used and disclosed as well as how you can access your information. Please review this notice in its entirety as the privacy of your medical information is important to us.
WHO FOLLOWS THIS NOTICE:
This notice describes the privacy practices of Fast 602 LLC and the independent professional health care entities ("Medical Professionals") contracting with Fast 602 LLC. The Medical Professionals provide the requested healthcare services, with which Fast 602 LLC facilitates coordination. Fast 602 LLC does not provide any direct medical services, nor does it refer or recommend any specific physician or medical practice or any other health care provider or personnel directly for treatment.
- Receiving an electronic or paper copy of your medical record: Following completion of our services, you can ask for an electronic copy of the relevant medical records. We will provide a copy or summary of your health information in a time frame compliant with local and state requirements, generally within 15 days of your request.
- Asking to correct your medical record: You may ask to correct your health information that you think is incorrect or incomplete. Upon receipt of your request, we will assess the changes and provide you a report within 60 days of what can or cannot be changed.
- Requesting confidential communications: You may ask us to contact you differently from your primary communication method. We will comply with all reasonable requests.
- Asking to limit what Fast 602 LLC uses or shares with others: You may ask us not to use or share your health information. However, should your request negatively affect the service provided or violate any local, state, or federal law, we can discuss the rationale and any risks.
- Providing you a list of those with whom we’ve shared information: You may ask for a list (accounting) of the times we’ve shared your health information. We will keep records for no greater than six years. The list will include the individuals with whom we shared your information and the reason for sharing.
- Get a copy of this privacy notice: You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
- Choosing someone to act for you: If you have provided someone with a medical, limited, or full power of attorney or if someone is your legal guardian, that individual may exercise some of your rights and make certain choices about your health information. Please provide us a copy of this information to ensure proper authorization.
- File a complaint if you feel your rights are violated: You can file a complaint if you feel we have violated your rights by contacting us using the information below. For certain health information, you can tell us your choices about what and how we share. Please contact us to provide your instructions.
- Sharing information with your family, close friends, or others involved in your care.
- Sharing your information for Marketing purposes.
- Fundraising efforts by Fast 602 LLC.
- Sharing information in a disaster relief situation.
- Sharing your information with emergency health care professionals, if there is a serious and imminent threat to your health or safety.
We typically use or share your health information in the following ways:
- Other health professionals: We may share your information with other health professionals involved in the resident's care, such as the resident's Primary Care Physician, as necessary for the completion of the 602a form and related services.
- Internal usage for process improvement: We may use your information to help us improve our services. All information used for this purpose will be deidentified to help protect your identity.
- Billing for services: We may use and share information as necessary to bill for our services.

The following conditions may occur for Fast 602 LLC to share your information with public entities or for research:
- Disease prevention.
- Product recalls.
- Adverse reactions to medications.
- Reporting suspected abuse, neglect, or domestic violence.
- Preventing or reducing a serious threat to anyone’s health or safety.
- Respond to organ and tissue donation requests: We can share health information about you with organ procurement organizations.
- Medical examiner or funeral director: We can share health information with a coroner, medical examiner, or funeral director should there be a death during our services.
- Address workers’ compensation, law enforcement, mandated reporting and other government requests: We may use or share health information about you for workers’ compensation claims; for law enforcement purposes; with health oversight agencies for activities authorized by law; for special government functions such as national security; and for any applicable mandated reporting purposes such as child abuse or sexual assault.
- Respond to lawsuits and legal actions: We can share health information about you in response to a court or administrative order, or in response to a subpoena.
We are required to be more restrictive in our sharing of certain types of information, such as psychotherapy notes, behavioral health information, genetic testing information, information on persons with developmental disabilities, HIV/AIDS testing information, and alcohol and drug abuse treatment. If Fast 602 LLC encounters this information, we will follow all local, state, and federal laws in sharing such information.
Fast 602 LLC is required by law to maintain the privacy and security of your protected health and personal identifiable information. We are required to contact you promptly if a breach occurs, which may have compromised the privacy and security of your information. The Privacy Policy describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. You acknowledge you have received a copy of our Privacy Policy and have been provided an opportunity to review it, and consent to receipt of an electronic copy.
You consent to the use of unsecured email, mobile phone text message, or other electronic methods of communication (“E-messages”) between yourself and Fast 602 LLC, your treating provider, and any other agents or representatives involved in your service request, for purposes of discussing material relevant to your service or health records. You understand that E-Messages are typically not confidential means of communication and that there is a reasonable chance that a third-party may be able to intercept and see these messages. You have been informed of the risks of transmitting your protected health information by an unsecured means. You acknowledge that E-messages are not to be used in the case of an emergency, and that you should call 911 or proceed directly to the nearest emergency room.
You acknowledge you have read, understand and agree to the information below, which applies if you have requested telehealth services for a resident, and that the resident’s name and identity have been correctly identified in communications with the medical professional.
You hereby consent to the resident receiving assessment through telehealth from the medical professional as part of the 602a form evaluation. You further give the medical professional permission to consult with relevant specialists as needed during the course of the assessment, and you further consent to the medical professional forwarding medical information to the resident’s primary care provider/provider of record (if not the medical professional) or, upon your request, to any other provider.
You are providing the foregoing consents based on your understanding of the following:
- During a telehealth assessment, the medical professional and resident will be in different physical locations and medical information will be communicated electronically.
- The use of telehealth may present certain risks, such as delays due to technological issues, the need to reschedule if transmitted information is of insufficient quality, or failure of potential security protocols which could cause disclosure of personal information.
- A lack of access to complete medical records could result in unintended issues; it is your responsibility to share complete and accurate information.
- The medical professional's information (name, degree, specialty, license, etc.) is available upon request. In the event of an inability to communicate due to technological failure, you may contact Fast 602 LLC support for assistance.
- The medical professional has the sole responsibility and discretion to determine whether telehealth is appropriate for the assessment.
- You have the right to withdraw consent to the use of telehealth at any time without affecting the resident's right to future care or treatment.
- Information disclosed during telehealth will be integrated into the resident's record and generally protected and confidential. Fast 602 LLC uses secure technology compliant with federal privacy laws, incorporating reasonable and appropriate network and software security protocols (including multi-factor authentication and AES256 encryption). However, there is a risk that data security protocols could fail or be breached.
- Fast 602 LLC will not provide personally-identifiable information to any third parties without express consent, except for scheduling, billing, and other treatment, payment, and health care operations purposes, or other uses/disclosures permitted or required by law.
- Fast 602 LLC and its facilitated medical professionals are not responsible for any information lost as a result of technical failures during telehealth.
- If a medical emergency is experienced, you will be directed to call 911; Fast 602 LLC is not able to connect directly to local emergency services.
You have discussed the foregoing information with your facility team and all of your questions have been answered to your satisfaction.
You consent to the receipt of communications about other healthcare products or services offered by Fast 602 LLC or its partners and affiliates. These communications may be received in several formats including electronic, SMS, and postal mail.
Fast 602 LLC may share certain protected health information with partners and affiliates that provide, arrange, or offer other healthcare-related services. These partners and affiliates may also receive your information to offer other products and services which may be beneficial to you. By proceeding with your selected services, you are agreeing to have your information shared for internal marketing purposes by Fast 602 LLC, as well as external marketing purposes with companies and affiliates with whom Fast 602 LLC works. These communications may be received in either electronic format or postal mail. Please note that you may revoke this consent at any time.
You acknowledge that you (1) have read, understand and accept these Terms of Service; (2) have received a copy of Fast 602 LLC's Privacy Policy and further acknowledge that it is available upon request; (3) consent to the use of E-messages between your facility and Fast 602 LLC, the facilitated medical professionals, and/or other agents or representatives, for purposes of discussing material relevant to your service requests or resident records; and (4) have read and understand the information contained in the Consent to Use of Telehealth above, and are providing the consents expressly set forth therein.
If an RCFE administrator is unable to consent, you acknowledge that you agree to the terms and conditions of this agreement as the legally authorized representative of the facility.
Fast 602 LLC’s Commitment to Nondiscrimination and Equal Access to Services
Fast 602 LLC does not discriminate, and we comply with all applicable anti-discrimination and equal protection laws. This means we do not discriminate against, exclude, or treat differently any individual or Residential Care Facility for the Elderly (RCFE) based on their race, color, national origin, disability, religion, marital status, veteran status, sexual orientation, gender expression, gender identity, age, or status in any other legally protected class.
Ensuring Equitable Access to Services
Fast 602 LLC is committed to ensuring equitable and non-discriminatory access to our services, accommodating the needs of residents and facilities. Our digital and telehealth-centric service delivery model improves access by alleviating the need for physical transportation or ensuring specific on-site accommodations at a medical office.
We work with RCFEs and resident representatives to identify and facilitate available, commercially reasonable accommodations that could improve a resident’s access to the telehealth consultation and/or quality of service. Where possible, we will assist in providing or coordinating accommodations including, but not limited to:
- Qualified sign language interpreter services.
- Qualified interpreters for residents whose primary language is not English.
- Communication support for residents with disabilities.

Our Cancellation Policy outlines the terms and conditions for canceling 602a form service requests with Fast 602 LLC. We aim for fairness and transparency in all our procedures.
All cancellations for a 602a form request, including the associated telehealth consultation and TB test coordination, must be submitted by sending an email to support@fast602.com at least 2 hours prior to the scheduled telehealth consultation time. You will receive a complete refund if the cancellation is completed in accordance with this timeframe.
If you decide to cancel a 602a form request, including the telehealth consultation, less than 2 hours prior to the scheduled consultation time, a cancellation fee of $50 will be charged.
In the event that the resident requiring the 602a form utilizes 911 services or seeks care at a hospital emergency room/urgent care center prior to the scheduled telehealth consultation or prior to the finalization of the 602a form by our medical professional, the entire service fee will be refunded.
Fast 602 LLC is fully committed to maintaining the privacy and security of Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Our platform and operational procedures are designed to meet all HIPAA requirements, including:
Administrative Safeguards: We have implemented robust policies and procedures, security management processes, information access management, and workforce training to ensure compliance and protect PHI. Our team members are regularly trained on HIPAA regulations and best practices.
Physical Safeguards: We employ measures to protect our physical infrastructure and equipment that store or transmit PHI. This includes secure facility access controls and workstation security.
Technical Safeguards: Our systems utilize advanced technical security measures such as access controls, audit controls, integrity controls, and transmission security (encryption) to prevent unauthorized access, modification, or disclosure of electronic PHI.
Business Associate Agreements (BAAs): We enter into BAAs with all third-party vendors and partners who may come into contact with PHI, ensuring they are also contractually obligated to uphold HIPAA standards.
Breach Notification: In the event of a suspected breach of unsecured PHI, Fast 602 LLC has established clear protocols for detection, containment, and notification as required by HIPAA.
