What documents do I need to provide for the 602a form?

You will need to securely upload the resident's Health & Physical (H&P) reports or Discharge (DC) summaries from a hospital or skilled nursing facility. These documents provide the comprehensive medical history necessary for our professionals to accurately complete the 602a form.

How long does it take to get a completed 602a form?

Our streamlined process is designed for efficiency. Depending on when the request is submitted, a version of the 602a form without complete TB clearance can often be available within 24 hours. The biggest potential hangup in the full completion of the form is the Tuberculosis (TB) clearance. If a resident has not had a TB clearance within the last three months, a new clearance will need to be obtained, which can extend the overall timeline. Our team will coordinate this process to ensure timely completion.

Is a telehealth visit with the resident required?
